Guidelines for completing the request for insertion form for non-federal government clients

Sections 1, 2 and 3 are mandatory.

Section 1: Client information

Requesting organization’s name
Indicate the legal name of the company, group or association or the name of the individual required to publish a notice pursuant to an act or regulations.
Address
Provide the complete mailing address of the contact person for the notice.
Contact name
Provide the name of the person whom the Canada Gazette Directorate should contact to acknowledge receipt of the notice and for questions about the content, etc.
Telephone and extension numbers
Indicate the telephone number and extension number (if applicable) of the contact for the notice.
Email
Please provide the email address of the contact person for the notice.

Section 2: Notices

Title of document submitted
Indicate clearly the title of your notice.
Date submitted
Indicate the submission date of your notice to the Canada Gazette Directorate.
Published under the authority of (section of Act, Regulations, etc.)
Indicate the name of the act or regulations under which your notice must be published. Please note that all notices appearing in the Canada Gazette must be published in accordance with a federal act or regulations.
Expected date of publication
Indicate the expected date of publication. Refer to the Deadline Schedule for the list of publication dates and related deadlines. Some deadlines vary due to statutory holidays.

Section 3: Invoicing contact information

Organization’s name
If different from the requesting organization’s name, indicate the legal name of the company, group or association responsible for sending the notice to be published to the Canada Gazette Directorate and to whom the Canada Gazette Directorate will send the invoice.
Address
Provide the complete mailing address where invoices can be sent should there be no email address.
Contact name
Indicate the name of the person to whom all questions related to your invoice should be addressed.
Business number (mandatory)
Provide the Business Number of your company, group or association. A Business Number is a nine-digit business identifier used in Canada to which businesses can register program accounts with the Canada Revenue Agency. For more information on the Business Number, please consult the Canada Revenue Agency's Web site at www.cra-arc.gc.ca/tx/bsnss/tpcs/bn-ne/wrks-eng.html.
Telephone and extension numbers
Provide a telephone number and extension number (if applicable) of the person who will answer all questions related to your invoice.
Email
Provide the email address of the contact person who will answer all questions related to your invoice.
Do you need a legal affidavit ($22)?
A legal affidavit could be used as an official proof of the publication of your notice. It is available upon request.
Do you need translation?
The Canada Gazette Directorate offers translation services, for a fee, for 3 types of notices: plans deposited, surrender of charter and relocation of head office. Contact us for further information on the costs associated with these services.
How many times must your notice be published?
Most notices need only be published once; however, some acts and regulations require that certain types of notices be published 4 times.
